System Forms

For Building Block 1, there are several printed forms:

• INVOICE
• STATEMENT
• MAILING LABEL

The standard invoice and statement forms have been designed for letter 8.5 x 11-inch size paper printed in portrait mode. The standard mailing labels can be one-up 1 x 3.5 inch labels or the full page (Avery 8160) with 3 across by 10 down. These were selected because of their widespread availability and low cost.

You can request custom setups for any of these forms. These can vary in content, paper size, portrait vs. landscape orientation, graphics for logos, fonts and font sizes, colors, and barcodes, which will be dependent on the capabilities of the printer you will be using. You can also choose between plain paper or preprinted forms. There is an hourly charge for customization.

For Building Block 2, the standard WORK TICKETS are printed on plain 8.5 x 11 inch paper and typically folded in half to fit in the case pan.

Customization of work tickets is available as described above...we have found that labs want to use various colors and print fonts and sizes to distinguish task centers and dates so that certain case information can be discerned from a distance.

Another special feature available for work tickets is the use of barcodes to provide highly accurate and fast entry for case tracking and technician productivity in the Barcode Supplement.
